US CLUB REGISTRATION PROCEDURES

 

COACH REGISTRATION

  • Go to: www.usclubsoccer.org
  • Pick the Registration tab at the top right
  • Then select Staff Background Check
  • Click on the link to the Staff Background Check Application and follow the directions below.
    Please use the following responses:
  • State: Washington
  • Registration type: Coach / Assistant Coach / Manager
  • Type of membership: League Team
  • League: Oregon Premier League 
  • Club: Pacific FC 

As soon as you register as a coach, please send me:

  • Team Name:
  • Legal Name (the one you use on the staff background check):
  • Position: (coach, ass't coach, manager)
  • Address:
  • Phone:
  • Email:
  • $15 for each coach card to be ordered.  Card is good for 2 years. Mail payment to:

PLAYER REGISTRATION

If your team is going to play Portland Metro League (or any other WYS league), you should register with Rachelle through VanWest.

 

If your team is going to play Oregon Premier League (or wants US Club player cards), please keep reading.  Your parents will NOT need to login to our club website to register as they have done in the past.  Registration is done as a team with the procedures below.  

 

Here are the instructions:

 

For players that DO NOT already have Pacific FC Player Cards: I can order them a new card on June 1st for use in summer tournaments.   I will need:

1.    The team spreadsheet filled out. (1 per TEAM -- Form at:  http://www.pacificfc.org/teams/regforms)

2.    A scanned birth certificate for each player.
Send DIGITAL copies of these to me saved in the following way:  Player's last name, Player's first name, bc.  Example:  morganalexbc.pdf.  Paper copies are not allowed as they need to be submitted online with the application.  I have 400+ birth certificates to track, so please help me out with the saving of these.  :)

3.    A hardcopy OR scanned Concussion Compliance for each player.  
(1 per Player AND Coach-- if scanned save as last name, first name, conc.  Example:  morganalexconc.pdf)

4.    A hardcopy OR scanned Registration Form for each player
(1 per Player -- if scanned save as last name, first name, reg.  Example: morganalexreg.pdf)

5.    $100 for each player made payable to Pacific Soccer Club.

For players that ALREADY have current Pacific FC Player Cards (even if they were on a different PFC team last year), I can order them a new card on July 1st.  I will need:

2.    A hardcopy or scanned Concussion Compliance for each player.
(1 per Player AND Coach-- if scanned save as last name, first name, conc.  Example:  morganalexconc.pdf)

3.    $100 for each player made payable to Pacific Soccer Club

Team Managers should keep a binder of Registration Forms and Concussion Form copies.  Most tournaments require these items at check-in.

Cards can take 7-10 days to be received after they are ordered, please give yourself adequate time to get them before tournaments!
